Job Description: House Call Nurse Practitioner (NP), Primary Care

The House Call Nurse Practitioner, specializing in primary care, will work closely with the Medical Director (MD) to provide excellent clinical services to various IntraCare House Call Patient. They will direct, implement, and provide a plan of care for patients. They will provide general preventative care, conduct check-ups, treat both acute and subacute and chronic illnesses, order lab tests, and prescribe medication for patients.

House Call Nurse Practitioner will make house calls throughout the DFW metroplex. The majority of patients are elderly with Medicare coverage and are assigned to the nurse practitioner for continuing primary care visits every 30-60 days. 

Visits include but are not limited to taking vital signs, performing an exam, ordering labs/treatments as necessary, diagnosing, managing chronic conditions, prescribing, and charting the visit. 

The Nurse Practitioner will report to the Market Director, Clinic Operations and will receive clinical direction and oversight from the Medical Director.

Key Responsibilities:

The ideal candidate would be experienced in performing physical exams and patient observations, recording patients' medical histories and symptoms, creating patient care plans and contributing to existing ones, ordering, administering, and analyzing diagnostic tests, monitoring and operating medical equipment, diagnosing health issues and plans of care for patients, administering medicine and other treatments, detecting changes in patients' health and modifying treatment plans (as needed), consulting with healthcare professionals and families, training patients and their families to manage and prevent illness and injury, prescribes appropriate pharmacologic and non-pharmacologic treatment modalities, facilitates the patient’s transition within and between health care settings, e.g. admitting, transferring, and discharging patients, facilitates staff, patient and family decision making by providing educational tools, monitor and provider start-up status of clinics, and all other duties as assigned.

Ideal Candidate: 

The ideal candidate would have prior experience working within a general practice setting or an advanced practice setting such as cardiology, pulmonary, endocrinology, and/or neurology.

The ideal candidate would have experience working with an aging population with multiple co-morbidities.

Candidates who might have worked in other countries, in a status equal to or above the US Nurse Practitioner role such as a physician would be highly considered based on the amount of advanced knowledge that they bring to the table.

This candidate should have a clear license to practice in the United States, specifically in Texas.

A candidate with prior house call experience would be preferred.

Required Skills and Abilities:

This candidate must maintain high ethical standards and display professionalism at all times. This candidate must have the ability to multitask, make complex decisions within their scope of practice, display a level of confidence in care, quickly identify changes in conditions and take the appropriate action needed based on the situation.  This candidate should have excellent communication skills, have attention to detail, and accurately document within the EMR.

Education & Experience:

  • Graduate of an accredited Nurse Practitioner Program
  • Current APRN/Nurse Practitioner Licensure in State of Texas
  • Current Board Certification in adult primary care or family practice specialty area
  • Current Medicare provider enrollment with active PTAN number
  • Current TX Medicaid provider enrollment with active TPI number
  • Current provider card in Basic Life Support (BLS)
  • Current provider card in Advanced Cardiac Life Support (ACLS), or willingness to obtain upon hire
  • Current DEA License, or ability to obtain DEA license upon hire

Physical Requirements:

  • Must have access to reliable transportation and ability to commute to the clinic location

Position Details:

  • Willing to travel to around the DFW Metroplex to patient homes.
  • Malpractice insurance coverage and all required supplies are provided. 
  • Use of a private vehicle must be available. (Paid Milage)
  • This is not annual in-home assessments/wellness visits.
